Mediclaim Facilities

 

For Planned / Non-Urgent Hospitalization:

  • It takes 2 to 3 days to get approval from the time the form is submitted.
  • Immediately after consultation – Contact the Insurance Desk.
  • Desk will help you complete the Form and will collect the necessary documents*.
  • Desk Timings: 10 am to 6 pm Monday to Sunday.

For Emergency Hospitalization:

  • Contact the Desk as soon as possible.

 

 

Points to be noted

  1. The Insurance Desk is only a facilitator. The Hospital cannot influence the Insurance company. The desk is only a conduit between you and the Insurance Company. TPA (Insurance Company) may not approve the cashless hospitalization for various reasons outlined below –
  • If the disease for which you are being admitted is not covered in the policy.
  • Information submitted in the Form is inadequate, for which further documents from

you or the treating Physician will be required. This is quite common.

  • If there is a waiting period in your policy.
  • If the sum insured is insufficient for the proposed treatment.

 

  1. Initial history declared by the patient to the doctor or in the Form will be considered as the truth and cannot be modified/changed later.

Quite frequently, the company representative will phone or visit the patient personally for verification. You are advised to stick to what you have declared. Any discrepancy will result in denial of the cashless treatment.

 

  1. Room category depends on the sum insured. Rooms with higher charges are generally disallowed by the TPAs. In case you opt for a room with a higher charge, then a substantial amount of the bill has to be additionally paid by you.

 

  1. Co-payment and Non-medical expenses are to be borne directly by the patient/patient relative during discharge clearance.

 

  1. The hospital will not provide breakup details of packages (The Insurance company disallows this).

All Investigations, reports or X-ray plates will be retained by the hospital in ORIGINAL, for submission to the Insurance Company. You can get a copy from the hospital on a nominal payment.

 

  1. In case of cashless denial from the TPA, the patient/patient’s family has to pay the total bill in cash to the hospital before discharge.

 

  1. Please allow sufficient time on the day of discharge to process your claim.

It takes at least 3 to 4 hrs. (starting from 10 am).

 

Insurance Documents Check List

For All policies (Retail & Corporate):

Xerox copy of Insurance card, current year valid Policy copy.

Patient’s Pan and Aadhaar.

Policy proposer’s (Main holder) PAN and Aadhaar.

All prescriptions and reports related to the current ailment.

Admission Advice

Colored passport photo of the proposer (Main holder) for KYC.

 

For Corporate policies:

Xerox copy of Employee ID card.
